Qualifications

• Bachelor or Master of Social Work or equivalent degree.
• At least one year of experience coordinating community-based services for older adults.
• Bilingual Spanish/English is required.

Responsibilities

• Provide care management for Eldersource clients.
• Accept referrals as assigned and maintain a caseload.
• Complete in-home assessments and create/modify care plans.
• Record case activity and provide information/referral counseling.
• Arrange mental health interventions and provide supportive counseling.
• Participate in team meetings and Lifespan trainings and QA activities.
• Maintain up-to-date professional knowledge in geriatric care and community services.
