Job summary

North Bay Regional Center seeks a Social Services Case Manager to coordinate services for regional center clients, including those with intensive needs. You will ensure the Individual Program Plan is met and facilitate access to resources and services in the community.

You will work in a role that requires in-person interactions; some teams may offer hybrid arrangements after an initial in-office period, with a focus on face-to-face client contact in Napa, Sonoma and Solano counties.

Qualifications

• Bachelor’s degree in a field related to developmental disabilities (Social Work, Psychology, Counseling, Special Education, Nursing).
• Master’s degree preferred.
• Minimum 3 years’ experience with individuals with developmental disabilities.
• Bilingual ability (Spanish to English) preferred.

Responsibilities

• Manage delivery of services to NBRC clients to meet Individual Program Plan objectives.
• Use person-centered planning to assist clients in accessing resources before using regional center funds.
• Monitor vendor services to ensure compliance with service authorizations.
• Evaluate and adjust Individual Program Plans and community resources.
• Conduct annual reassessments and client assessments as needed.
