Job summary

Grandsettlement in New York is seeking a part-time bilingual Spanish Case Worker to support homebound seniors through intake, case planning and referrals. You will coordinate services to reduce isolation and enhance wellbeing in a community-based program.

The role requires strong communication, empathy, and collaboration with a diverse team. A BA or Associate Degree in Human Services is preferred, with Spanish fluency essential for engaging participants.

Qualifications

• BA in Social Work, Psychology, or related field preferred or an Associate Degree in Human Services.
• Excellent communication, analytical and problem-solving skills.
• Experience delivering social services to seniors.

Responsibilities

• Provides case assistance and case management to participants, documents intervention in client files and enters data.
• Conducts initial interview and intake assessments to identify needs and formulate goals.
• Assists participants in obtaining services and entitlements (Medicaid, food stamps, SCRIE, home care referrals).
• Enrolls NDA clients with assessment and follow-up notes; enters data into funder database.
• Provides GMHI screening to support mental health awareness in the Baruch community.
• Coordinates program events and conducts home visits as needed.
• Provides translation services for nurse and counseling services.
• Assists in Food Distributions/Food Pantry as needed.
• Connects seniors to Grandsettlement Senior Center/NORC activities.
