Job summary

Sunnyside Community Services in New York is seeking a Bilingual Case Manager to help impaired older adults safely maintain independent life by linking them to community resources and home services, and advocating to obtain benefits.

You will work with clients and families to formulate appropriate care plans, conduct home visits, and maintain contact by phone. The position is hybrid: three days in office, two remote.

Qualifications

• MSW/BSW degree with two years of supervised human services experience or equivalent case management experience.
• Ability to work bilingual with older adults and families.
• Driver preferred.

Responsibilities

• Screen requests for services, determine eligibility, provide information and referrals.
• Conduct home assessments to evaluate living conditions and needed services.
• Maintain contact with clients, caregivers, and families, and coordinate care plans.
• Advocate for clients regarding benefits and medical care.
• Refer clients to emergency services when necessary.
• Develop and maintain relationships with community and government agencies.
• Maintain client/confidential records and participate in trainings.
