Job summary

The New York Foundling is seeking a Case Aide to support foster care services in a collaborative, interdisciplinary team. You will assist in promoting permanency for children and families, providing advocacy and referrals, and helping families with housing, benefits, and appointments.

Responsibilities include conducting workshops, transporting clients when needed, maintaining documentation, attending meetings, and building a regional resource guide to link families to services post-discharge.

Qualifications

• Prior experience and demonstrated skills working with children and families
• Listening, engagement, and basic supportive counseling skills
• Ability to work as part of a team
• Good organizational and writing skills

Responsibilities

• Work as part of an interdisciplinary team to support and expediate the service delivery to our youth and families.
• Provide support services to Families including advocacy, information sharing and resource referrals such as parenting skills, community MH services, HRA assistance, escorting to appointments, etc.
• Conduct workshops for parents with focus on training them on the completion of housing applications, food stamp and health benefits applications, and other applications for services.
• Responsible to assist Case planning team with driving agency vehicles to transport clients (children and families) to hospitals, residential settings, medical appointments, shopping emergency admissions and emergency replacement of clients etc.
• Assist Case Planning Team with the delivery of concrete services and supervising visits where appropriate.
• Responsible for submitting Visitation Notes, Meeting Minutes and all necessary paperwork in a Timely Manner.
• Responsible for attending Case Conferences, PPC, Treatment Team Meetings and Trainings as needed to support the Case planning team.
• Research and develop a regional resource guide of community programs/resources to link families up for services post discharge from the agency.
