Job summary

Advance Care Alliance New York is seeking a Care Manager to coordinate access to health and social services, develop Life Plans, and advocate for members’ needs. You will work with members, families, and providers to ensure integrated, person-centered support across HCBS and state programs.

The role involves regional travel, comprehensive assessments, and active collaboration within an interdisciplinary team to maintain eligibility, documentation, and timely follow-up.

Qualifications

• Bachelor’s degree with relevant experience, OR RN license with relevant experience, OR Master’s degree with relevant experience.
• Fluent understanding of community cultural traditions, norms, and practices of Members and families.
• Absolute integrity and commitment to serving people with I/DD and families.
• Excellent interpersonal, public speaking, and written communication skills.

Responsibilities

• Deliver person-centered care management in line with regulatory standards and policies.
• Complete comprehensive assessment/reassessment processes.
• Identify gaps in service provision and advocate to reach member goals.
• Develop and monitor Life Plans with interdisciplinary team input and timelines.
• Ensure timely service documentation and billing-ready records; maintain eligibility and proper consent.
