Job summary

Advance Care Alliance New York is seeking a Care Manager to coordinate access to health and community services for individuals with IDD. You will build person-centered Life Plans, advocate for members, and work with families, providers, and a care team to meet medical, behavioral, and social needs.

This role requires strong communication, autonomy, and the ability to travel within the designated service area to support members in achieving their goals and maintaining quality of life.

Qualifications

• Bachelor's degree with two years of relevant experience, or a RN with two years of relevant experience, or a master's with one year of relevant experience.
• Fluent understanding of community cultural traditions and practices of Members and families.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete comprehensive assessment/reassessment processes and identify gaps in service provision.
• Develop, implement, and monitor Life Plans with an interdisciplinary team, centered on the member.
