Qualifications

• A Bachelor of Arts or Science degree with two years of relevant experience, or a license as a Registered Nurse with two years of relevant experience, or a master’s degree with one year of relevant experience.
• Fluent understanding of community cultural traditions, norms, and practices of Members and their families.
• Absolute sense of integrity and personal commitment to serving people with I/DD and their families.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and in alignment with the agency's quality management plan, policies, and SOPs.
• Complete a comprehensive assessment/reassessment process.
• Identify gaps in service provision and make referrals; advocate to meet member goals.
• Develop, implement, and monitor Life Plans within required timeframes with an interdisciplinary team.
• Travel throughout the designated service area to meet with members and providers.
