Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete comprehensive assessments and reassessments.
• Develop, implement, and monitor Life Plans with an interdisciplinary team.
• Identify gaps in service provision and refer to appropriate services.
• Ensure accurate documentation and timely billing-related records.
