Job summary

Qualifications

• RN/LVN/LPN, MA, MSW or related clinical background with chronic conditions.
• Demonstrated clinical experience and patient-centered care planning.
• Bilingual or multilingual communication required; HIPAA compliance understood.

Responsibilities

• Drive high-volume outbound outreach to patients daily.
• Build genuine rapport and sustain ongoing patient relationships.
• Perform risk stratification and manage patient panels.
• Coordinate care plans with SMART goals and PCP engagement.
• Document conversations and actions clearly for the care team.
