Vigilance Health, Inc. is seeking a Care Manager / Patient Engagement Specialist to support patients through proactive outreach and compassionate care. This remote, full-time role requires bilingual communication and a patient-centered approach to health journeys.

You will manage a large patient panel, document care plans, and collaborate with care teams to improve outcomes, reduce ER visits, and maintain HIPAA-compliant records.

Qualifications

• RN, LVN, LPN, MA, MSW, LCSW, Medical Case Manager, Care Manager, Health Coach, Community Health Worker.
• Demonstrated clinical experience with patients with one or more chronic conditions.
• Bilingual or multilingual communication required.

Responsibilities

• Patient Outreach & Engagement: outbound calls 22+ connections daily by month three.
• Relationship Building & Rapport with patient panel using Motivational Interviewing.
• Risk Stratification & Panel Management to keep low-risk patients stable and reduce ER visits.
• Care Coordination & Goal Setting with SMART goals and patient-centered care plans.
• Documentation & Clinical Best Practices to accurately capture patient status and progress.
