Qualifications

• Must have clinical experience with one or more chronic conditions.
• Experience developing patient-centered care plans and SMART goals.
• Bilingual or multilingual communication required.

Responsibilities

• Make outbound calls 22+ daily to engage patients.
• Build rapport with patient panels using Motivational Interviewing techniques.
• Perform risk stratification and panel management to prioritize outreach.
• Coordinate care plans and goals with patients and PCPs.
• Document conversations clearly for the care team and care management records.
