Qualifications

• LVN/LPN license is required.
• Proven ability to build trust quickly over calls.
• Experience in high-volume outbound calling.
• Familiarity with Motivational Interviewing or quick uptake of frameworks.
• Strong listening skills and clear documentation.
• Healthcare or care management experience a plus.

Responsibilities

• Patient Outreach & Engagement: drive outbound calls and connect with patients daily.
