Qualifications

• RN, LVN, LPN, MA, MSW, LCSW or Medical Case Manager with active license
• Experience with chronic condition management and care planning
• Bilingual in English and another language preferred
• Proficient in documenting and coordinating care
• Strong interpersonal and motivational interviewing skills

Responsibilities

• Conduct high-volume patient outreach and engagement calls daily.
• Build rapport with patients using Motivational Interviewing techniques.
• Perform risk stratification and manage care panels.
• Coordinate care plans with PCPs and track SMART goals.
• Document conversations clearly for the care team.
