Epiphany Family Services seeks a Bilingual Care Manager Extender to join our Tailored Care Management program in the Raleigh/Durham area. The role supports care managers across physical, behavioral health, pharmacy, LTSS, and resource needs.

The position offers hybrid in-office, field, and remote work with flexible hours to meet client needs and travel within service areas as required.

Qualifications

• Must be at least 18 years old.
• Must have a high school diploma or equivalent.
• Trained in Tailored Care Management as described in the document.
• Supervised by a TCM Care Manager and meet one of: lived experience with behavioral health condition with a Certified Peer Support Specialist; or one year of paid relevant experience.

Responsabilités

• Share personal story to assist and support the member.
• Perform general outreach, engagement, and follow-up with members.
• Coordinate services and appointments including reminders and transportation.
• Engage in health promotion activities per Tailored Care Management guidelines.
• Share information with the care manager and team on member circumstances.
• Provide and track referrals and assist with community resources.
• Participate in case conferences.
• Support assessment and addressing unmet health resource needs.
• Maintain regular communication with Care Manager to reduce duplication.
