Job summary

Qualifications

• Must be at least 18 years old.
• High school diploma or equivalent.
• Experience with I/DD/TBI or CFSP preferred.
• Must possess a valid driver's license.
• Knowledge of Cherokee culture preferred.

Responsibilities

• Assist with person centered planning, referral and linkage, and monitoring and follow-up.
• Ensure care plan activities are carried out under supervision.
• Document information in EHR and care management platform.
• Engage members in person centered planning to reach outcomes.
• Link clients to needed services and supports.
• Provide timely updates to care team on member status.
• Coordinate discharge planning and follow-up care.
• Attend staffing huddles and participate in quality improvement.
• Conduct home visits with patients and staff when needed.
• Arrange transportation and appointments, including reminders.
