Hope Services, LLC in Raleigh, NC seeks an Extender to support outreach, engagement, and care coordination for children, adolescents, and families. The role emphasizes health promotion, referral tracking, and collaboration with Care Managers to address unmet health-related resources.

Spanish-speaking candidates are preferred, and experience in peer-support is valued. This is a full-time, direct-care position.

Qualificações

• Must be at least 18 years old and have a high school diploma or equivalent.
• Lived experience with I/DD or TBI and knowledge of NC Medicaid system.
• Certified Peer Support Specialist with behavioral health lived experience.
• Parent/guardian with ≥2 years direct care experience (not for their own child).
• Spanish speaking/bilingual preferred.

Responsabilidades

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments.
• Engage clients in health promotion activities.
• Share information with Care Manager on the member's circumstances.
• Make and track referrals.
• Participate in case conferences.
• Support the care manager in assessing and addressing unmet health-related resource needs.
