Qualifications

• RN licensed and eligible to practice
• Experience in chronic care management and discharge planning preferred
• Ability to work with multidisciplinary teams and caregivers
• Fluency in English and Spanish required

Responsibilities

• Oversee chronic care and transitions of care for high‑risk patients within care teams.
• Serve as a resource for complex patient management and care plans.
• Triage via phone and route issues to appropriate party for intervention.
• Ensure efficient patient transitions from acute to home or transitional care.
• Conduct comprehensive assessments addressing physical, mental, and social risk factors.
• Coordinate with providers and care team to remove barriers to care delivery.
