Job summary

Housing Works Inc. is hiring a Care Manager in New York to support individuals with complex medical, behavioral health, and social service needs.

You will conduct in-person assessments in home, community, or office settings and develop person-centered care plans with clients and providers. This role emphasizes regular field presence, collaboration with multidisciplinary teams, and direct client engagement to navigate systems and improve outcomes.

Qualifications

• Bachelor’s Degree with 2 years related experience or a Master’s Degree with one.

Responsibilities

• Conduct comprehensive in-person initial assessments and annual reassessments.
• Develop and maintain individualized plans of care with input from clients and providers.
• Collaborate with medical and behavioral health providers through face-to-face meetings.
• Engage clients in person to review housing options and assist with housing applications.
• Coordinate client services with internal and external providers.
• Document all activities and outcomes in the case record.
• Maintain confidentiality in all client interactions.
• Perform other duties as assigned.
