Job summary

TimeDoc Health, Inc. is seeking a Bilingual Care Coordinator for a fully remote, full-time role. You will manage Chronic Care Management calls, document interactions in EMR, and coordinate care with providers while meeting daily productivity metrics.

The ideal candidate has CMA/RMA or LPN credentials and strong English communication, with Spanish language skills considered a plus. This remote position requires a quiet home office and reliable internet.

Qualifications

• Experience as CMA/RMA or LPN with active license.
• Strong English verbal and written communication skills.
• Ability to work remotely with structured schedules.
• Experience with CCM or chronic care management is a plus.

Responsibilities

• Conduct monthly CCM calls with assigned patients.
• Make 30–50 outbound calls per day.
• Document patient interactions to meet CCM goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with providers, pharmacies, and resources.
• Identify gaps in care and social determinants of health.
• Maintain HIPAA compliance and confidentiality.
