Job summary

TimeDoc Health is seeking a bilingual Care Coordinator for a fully remote role, supporting chronic condition management across US time zones. The position emphasizes phone-based outreach, care coordination, and documentation while meeting productivity metrics in a remote setting.

The ideal candidate has CMA/RMA or LPN certification with 1–3 years of relevant experience, strong English communication, EMR proficiency, and a reliable home office setup.

Qualifications

• Minimum 1–3 years of CMA/RMA or LPN experience.
• Active certification or multistate license for LPNs.
• Strong verbal and written communication skills in English.
• Comfortable on the phone for most of the day.
• Experience with EMR/EHR systems.
• Organized, reliable and able to work independently in a remote setting.

Responsibilities

• Conduct monthly CCM phone calls with the assigned practice.
• Make 30–50 outbound calls per day.
• Complete required patient interactions to meet CCM service goals.
• Educate patients on chronic conditions and care plans.
• Coordinate care with provider offices, pharmacies, and community resources.
• Identify gaps in care and social determinants of health.
• Create and update personalized care plans in the EMR.
• Accurately document all patient interactions.
• Maintain HIPAA compliance and patient confidentiality.
