Job summary

Person Centered Services seeks a Social Work Professional to operate as a Care Coordinator within a health home model for people with intellectual and developmental disabilities. The role centers on coordinating health, behavioral healthcare, and community supports with a client-centered approach and advocacy for each individual.

Responsibilities include life-planning, enrollment, home visits, and collaboration with a team to ensure needs are met in a respectful environment.

Qualifications

• Bachelor's degree with 2 years of relevant experience
• Licensed RN with 2 years relevant experience OR Master's with 1 year relevant experience
• Minimum Bachelor’s degree required by OPWDD in NYS

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support and their service team.
• Manage a tiered caseload while tailoring services to individual needs, similar to a Care Manager.
• Completes enrollment and eligibility documentation.
• Meets with individuals in homes, physician/provider offices, and other public places to conduct assessments and provide services.
• Commit to a respectful, just, and supportive environment aligning with diversity, equity, and inclusion.
