Endeavor DDD Services is hiring a Care Coordinator to oversee HCBS services for individuals with developmental disabilities in Phoenix. This in-person role reports to the Branch Manager and follows a Monday–Friday schedule from 8:30 AM to 4:30 PM.

The position offers a competitive pay range of $43,000 to $50,000 per year. The Care Coordinator will manage caseloads, ensure service plan alignment, conduct home visits, maintain compliant documentation, and liaise with families, DSPs, and state

• Clear, professional, empathetic communication across phone, text, email and in person.
• Strong attention to detail with the ability to manage multiple tasks, deadlines and documentation.

Responsabilités

• Caseload management of HCBS including Attendant Care, Habilitation and Respite.
• Ensure care matches authorizations and PCSPs, track goals and outcomes.
• Conduct regular home visits to assess environments and needs.
• Serve as the liaison between families, DSPs, state coordinators and staff.
• Assist DSPs by answering questions and resolving service delivery issues.
• Maintain compliant documentation per DDD, AHCCCS and HIPAA standards.
• Address gaps in coverage and advocate for available resources.
