Qualifications

• QMHP qualifications under Texas standards.
• Certified in CANS and/or ANSA.
• Bilingual in English and Spanish.
• Bachelor's degree in Human Services, Social Work, Psychology, Counseling, Sociology, or related field.
• Prior case management experience required.
• Experience with CANS/ANSA completion.
• Strong organizational and communication skills.
• Ability to work independently and meet deadlines.

Responsibilities

• Complete CANS/ANSA assessments accurately and timely.
• Coordinate care and connect clients to community resources.
• Maintain communication with families, providers, and referral sources.
• Assist with service planning and treatment coordination.
• Ensure documentation meets Medicaid, Texas, and agency standards.
• Monitor client progress and identify service needs.
• Maintain confidentiality and HIPAA compliance.
• Support office operations and intake workflow as needed.
