Job summary

Bristol Hospice LLC is seeking a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families and caregivers for up to one year after a patient’s death. The role emphasizes compassionate, team-based care within our hospice ecosystem and includes collaboration with personnel and community partners.

Qualifications

  • Bachelor's degree or equivalent experience or theological degree, OR master's in social work.
  • Experience with families/caregivers and patients through death/dying issues is required.
  • Ability to coordinate bereavement services within an interdisciplinary team.

Responsibilities

  • Coordinate the assessment and delivery of grief counseling (one-on-one, groups, follow-up) for families/caregivers.
  • Plan bereavement program development, expansion, and annual evaluation of services.
  • Provide bereavement support to hospice personnel coping with work-related grief; supervise interns and refer to resources.
  • Provide bereavement information and referrals to community callers.
  • Expand and maintain community resource lists as needed.
  • Assist in evaluating and planning services for personnel support needs (retreats, weekly groups).
  • Participate in hospice activities and advance professional knowledge via ongoing training.
