Job summary

Bristol Hospice-eugene Llc in Oregon seeks a compassionate Hospice Social Worker to support patients and families at the end of life. You will assess psychosocial needs, coordinate with the interdisciplinary team, and guide advance directives, funeral planning, and community referrals.

The role requires a MSW, two years in health care, willingness to travel, and the ability to educate others on cultural considerations.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Two years of documented supervised experience in health care; hospice experience preferred.
• Certified Hospice and Palliative Social Worker desirable.
• Willingness to travel throughout the Bristol Hospice service area and flexible hours.

Responsibilities

• Assess psychosocial status of patients and families and report findings to the team.
• Carry out social evaluations including family dynamics and risk indicators.
• Counsel patients and families; provide crisis intervention when necessary.
• Educate team on cultural needs, communication, and family roles.
• Maintain clinical records and assist with advance directives.
• Provide information and referrals to community resources.
• Participate in care planning and interdisciplinary group meetings.
• Assist with funeral arrangements and related decisions.
