Job summary

Bristol Hospice in Illinois seeks a Clinical Supervisor to oversee clinical staff and ensure the delivery of compassionate hospice care. You will assign case referrals, participate in care planning, and collaborate with medical leadership to ensure eligibility and quality of service.

The role emphasizes adherence to professional standards, staff development, and supporting patients and families through the hospice journey.

Qualifications

• A Registered Nurse license in good standing to practice professional nursing.
• Two years of management or supervisory experience in hospice/home care.
• Excellent observation, verbal and written communication skills.
• Current CPR certification.
• Valid driver’s license.

Responsibilities

• Receive referrals and assess patient information for hospice/home care needs.
• Assign appropriate hospice personnel to referral cases as needed.
• Consult with Medical Director and Attending Physician on eligibility for services.
• Review cases through home visits, records and clinician services.
• Guide clinicians to improve performance and delivery of quality home care.
• Be available during operating hours to assist clinicians as appropriate.
• Assist in screening and interviewing new employees; aid orientation.
• Support planning, implementation and evaluation of education programs.
• Contribute to policy development and organizational goals.
• Promote quality and customer service in hospice delivery.
• Participate in quality assessment and performance improvement teams as directed.
