Responsibilities

• Assess the psychosocial status of patients and families related to terminal illness and communicate findings to the interdisciplinary team.
• Carry out social evaluations including family dynamics, caregiver abilities, and plan interventions.
• Counsel patients and families in relation to stress and coping, providing crisis intervention when necessary.
• Educate the interdisciplinary group on cultural needs and traditions of patients and families.
• Maintain clinical records on all patients referred to social work.
• Provide information and referrals for practical and environmental needs.
• Serve as liaison between patients, families, and community agencies.
• Participate in development of individualized care plans and attend interdisciplinary meetings.
