Job summary

Bristol Hospice is seeking a compassionate Registered Nurse (RN) to perform comprehensive patient and family assessments and deliver high-quality hospice care across our California service area.

The role emphasizes nursing process application, care planning, and collaboration with an interdisciplinary team to meet terminally ill patients' needs while educating families in self-care and symptom management.

Qualifications

• One year of acute care experience in an institutional setting.
• Must understand the philosophy of hospice care and the needs of terminally ill patients.
• Willingness to travel throughout the Bristol Hospice service area.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family.
• Provide physical assessment and history of illness; develop a plan of care.
• Record pain and symptom management and outcomes; adjust plan as needed.
• Communicate changes to physician and obtain orders as required.
• Teach patient and family self-care techniques and involve them in planning.
• Collaborate with interdisciplinary team to coordinate care.
