Bristol Hospice is seeking a compassionate Hospice Social Worker to assess psychosocial needs, provide counseling, and coordinate support for patients and families at the end of life. You will work with an interdisciplinary team to ensure dignity and comfort in care, while addressing cultural and spiritual considerations.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Two years of supervised experience in health care, hospice experience preferred.
• Certified Hospice and Palliative Social Worker desirable.

Responsibilities

• Assess the psychosocial status of patients and families/catients and communicate findings to the nurse and team.
• Provide counseling to patients and families to address stress and coping.
• Educate interdisciplinary team on culture and family dynamics related to care.
• Maintain clinical records for referrals and care plans.
• Assist in planning for advanced directives and funeral arrangements as needed.
• Coordinate information and referrals with community agencies and resources.
