Job summary

Ascension is seeking a licensed clinician to perform high-level diagnostic assessments and develop individualized treatment plans within the Chemical Dependency department at our Elk Grove Village facility. You will deliver evidence-based individual, group, and family therapy, coordinating with a multidisciplinary team to monitor patient progress.

The role requires a Master’s degree and IL licensure, with preference for CADC and LPC/LSW credentials.

Qualifications

• Licensure / Certification / Registration: One or more of the following required: Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date; Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date; Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date; Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Education: Master’s degree required.
• Additional Preference: Master’s Degree/LSW or LPC required; Experience with chemical dependency preferred; CADC certification preferred.

Responsibilities

• Clinical Assessment & Diagnosis: Perform high-level diagnostic assessments to evaluate a patient's clinical needs and psychosocial status.
• Treatment & Psychotherapy: Develop individualized treatment plans with measurable goals. Administer evidence-based individual, group, and family therapy tailored to the patient's specific disorder or disability.
• Multidisciplinary Collaboration: Partner with a multidisciplinary team to align on treatment goals and patient progress. Lead or participate in care conferences, providing expert insights based on assessments and clinical updates.
• Schedule: 4 10-hour shifts and every third weekend.
