Job summary

Qualifications

Responsibilities

• Provides direct and indirect patient care under RN/LPN direction.
• Monitors vital signs, intake/output, and documents in EHR.
• Observe behavior and mental status, escalate changes to RN.
• Assist with ADLs, transfers, and meal services.
• Participate in safety, de-escalation, and CPI-related procedures.
• Contribute to treatment team meetings and care planning.
• Support orientation of new staff and patient education groups.
