Job summary

Oak Street Health invites applications for a Behavioral Health Specialist. You will provide care management and evidence-based short-term psychotherapy, coordinating with Care Teams and a psychiatric consultant.

Requirements include a Master’s in a mental health field, highest clinical license, US work authorization, and the option for remote work days based on program metrics. This is a full-time, benefits-rich role.

Qualifications

• Master’s Degree in the field of mental health counseling/healing arts required from an accredited school.
• Highest clinical level of license (or local equivalent) required.
• Experience with screening for mental health/substance use disorders.
• Experience with assessment and treatment planning.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral).
• US work authorization is required.

Responsibilities

• Collaborate with primary care teams to refer patients to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth sessions.
• Document encounters in the electronic medical record.
• Support regional/organizational efforts to advance the program.
