Job summary

Oak Street Health is seeking a Behavioral Health Specialist to deliver care management and short-term psychotherapy across a network of primary clinics. You will work with Care Teams, coordinate with a psychiatric consultant, assess patients, and refer to resources as needed.

The role requires a Master’s degree and highest clinical licensure; remote work days may be available based on program metrics and performance. 40 hours weekly, full-time position.

Qualifications

• Master's degree in mental health counseling or related field from an accredited program.
• Highest clinical license level (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, CBT).
• US work authorization.

Responsibilities

• Collaborate with care teams to identify appropriate patients for the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth collaboration.
• Document patient encounters accurately in the electronic medical record.
• Engage in regional efforts to advance the Behavioral Health Program.
