Oak Street Health is seeking a Behavioral Health Specialist to deliver short-term psychotherapy and care management within primary care teams. The role focuses on coordinating behavioral health care across clinics and collaborating with a psychiatric consultant to support patients.

The position requires a Master's in a mental health field and the highest clinical license (LCSW/LMHC/LMFT), US work authorization, and offers a hybrid work arrangement with potential remote days based on program

Qualificações

• Master's Degree in the field of mental health counseling or related discipline.
• Highest clinical license (or local equivalent).
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for these conditions.
• Familiarity with brief, structured interventions (e.g., MI, Behavioral Activation, CBT).
• US work authorization.

Responsabilidades

• Collaborate with primary care teams to refer patients to Oak Street's Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant, including telehealth handoffs.
• Develop relationships with primary care teams and communicate changes in care plans.
• Document encounters and counseling sessions in the EMR.
• Participate in regional and organizational efforts to advance the program.
• Possibly cover multiple clinics and perform other duties as assigned.
