Oak Street Health seeks a Behavioral Health Specialist to provide care management and evidence-based short-term psychotherapy to patients across primary clinics. The role emphasizes coordination with care teams and a psychiatric consultant, plus referrals to external resources as needed.

The position requires a Master’s in a mental health field and the highest clinical licensure, with US work authorization. A remote-work option may be available based on program metrics and performance.

Formación

• Master’s Degree in mental health counseling, social work, or related field.
• Licensed at the highest clinical level (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Responsabilidades

• Collaborate with primary care teams to identify appropriate patients for referral to Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and provide or facilitate referrals to Oak Street’s psychiatric consultant and/or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with Oak Street’s psychiatric consultant, including warm handoffs for telehealth visits.
• Develop relationships with primary care teams and communicate changes in care plans.
• Document patient encounters and counseling sessions in Oak Street’s EMR.
• Participate in regional and organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panel sizes.
