Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management services and short-term psychotherapy within our primary care clinics. You will coordinate care with care teams and a psychiatric consultant, assess patient needs, and make referrals to behavioral health resources.

This role may involve remote work one day per week, contingent on program metrics, and requires licensing and a Master’s degree in a mental health field with LCSW/LPC/LMFT credentials.

Qualifications

• Master's degree in mental health field from an accredited program.
• Highest clinical level of license (LCSW/LPC/LMFT) qualifying credential.
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health issues.
• Familiarity with brief, structured interventions (e.g., MI, Behavioral Activation, Problem Solving, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for the Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs and care plan collaboration.
• Maintain accurate documentation of encounters and counseling sessions in the EMR.
• Participate in regional/organizational efforts to advance the program.
