Job summary

Oak Street Health is seeking a Behavioral Health Specialist to deliver care management and evidence-based short-term psychotherapy across primary clinics. You will collaborate with Care Teams and a psychiatric consultant to assess needs, plan care, and coordinate referrals, including telehealth handoffs as needed.

Qualifications

• Master's degree in mental health counseling or a related field from an accredited program.
• Current license at the highest clinical level (or local equivalent).
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
• US work authorization.

Responsibilities

• Provide care management services and short-term psychotherapy to patients with behavioral health needs.
• Coordinate care with Care Teams and psychiatric consultant across a region.
• Assess patient needs and make referrals to behavioral health resources.
• Provide short-term counseling using evidence-based treatments.
• Document encounters accurately in the electronic medical record.
