Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy to patients with behavioral health needs across primary clinics. You will coordinate with care teams and a psychiatric consultant, assess needs, and refer to appropriate resources, delivering evidence-based treatments.

The role offers remote work days based on achievement and leadership decisions, with a 40-hour work week and a competitive benefits package.

Qualifications

• Master's degree from an accredited program in social work, counseling, marriage and family therapy, or a related field.
• Highest clinical license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiar with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the behavioral health program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs and care plans.
• Document patient encounters and counseling sessions in the EMR.
• Participate in regional/organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panels.
