Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy across primary care clinics. The role emphasizes coordination with Care Teams and a psychiatric consultant to assess needs and make referrals.

Requires a Master's degree in mental health counseling or related field and the highest clinical license, with experience in screening and treatment planning.

Qualifications

• Masters degree in mental health counseling or related field from an accredited program.
• Licensure at the highest clinical level (or local equivalent).
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured intervention techniques such as Motivational Interviewing, Behavioral Activation, Problem Solving, CBT.
• US work authorization required.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the behavioral health program.
• Develop care plans and coordinate with psychiatric consultants and external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits.
• Maintain accurate documentation of patient encounters and counseling sessions.
• Participate in regional and organizational efforts to advance the behavioral health program.
