Job summary

Oak Street Health is seeking a Behavioral Health Specialist to coordinate care management and provide short-term psychotherapy within primary care clinics. The role supports Care Teams, collaborates with a psychiatric consultant, and may include remote work days based on performance and program metrics.

Responsibilities include screening, care planning, referrals, warm handoffs for telehealth, and documentation in the EMR.

Qualifications

• Master's Degree in mental health counseling or healing arts from an accredited school.
• Highest clinical license level (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to Oak Street Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and provide referrals to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including telehealth handoffs and collaboration on care plans.
• Develop relationships with primary care teams and communicate changes in care plans.
• Accurate documentation of patient encounters and counseling sessions in the EMR.
• Participate in regional and organizational efforts to advance Behavioral Health Program.
• Possibly covering multiple clinics depending on panel sizes and maturity.
• Other duties as assigned.
