Responsibilities

• Provide case management and rehabilitative services to individuals, families and groups.
• Deliver crisis intervention services as directed by policy and protocol.
• Assess clients’ needs and help them function using client strengths and community resources.
• Complete all agency documentation accurately and timely.
• Assist clients to adjust and adapt to their environment, care and treatment.
• Maintain case management certification as required.
• Attend required meetings and meet performance expectations.
• Travel as needed for client transportation and related duties.
• Demonstrate professional judgment and effective communication with staff and clients.
