Zusammenfassung

GRAND Mental Health is seeking a Skills Development Professional to provide rehabilitation, wellness, and case management services to clients per their treatment plans. You will work under supervision to ensure service frequencies, high quality documentation, and billable service delivery, with emphasis on confidentiality and person-centered care.

The role requires collaboration with internal teams and external partners, strong communication, and adherence to professional boundaries.

Qualifikationen

• HS diploma or equivalent with 36 months direct experience with mental illness/substance abuse
• 60 college credits and 12 months behavioral health experience
• Bachelor's or Master's in any field with 6 months behavioral health experience
• Bachelor's or Master's in Behavioral Health
• Case Management II certification

Aufgaben

• Provide case management, wellness and rehabilitative services in various settings.
• Provide crisis intervention as directed by policy/protocol.
• Complete all agency documentation thoroughly and timely.
• Collaborate with treatment team members and manage care coordination.
• Travel up to 75% to community locations and client residences.
• Ensure billable services and fiscal accountability as designated by management.
• Utilize Eleos for eligible services.
• Participate in staff meetings and maintain professional standards.
