GRAND Mental Health in Stillwater, OK is seeking a Skills Development Professional to support outpatient treatment teams under supervision. You will deliver rehabilitative, wellness, and case management sessions per each client’s treatment plan, while maintaining professional boundaries and clear communication with clients, families, and providers.

• Must have demonstrated ability to evaluate client needs and plan effective interventions.
• Ability to link/referral and advocate with internal and external providers.
• Strong communication and professionalism under pressure.
• Commitment to confidentiality and ethical boundaries.
• Ability to work within a multidisciplinary outpatient treatment team.

Responsabilidades

• Provide case management and rehabilitative services to individuals, families and groups.
• Provide crisis intervention as directed by policy and protocol.
• Assess client needs and help them function using strengths and community resources.
• Complete all agency documentation accurately and timely.
• Assist clients to adapt to environment and treatment.
• Maintain case management certification as required.
• Attend staff meetings and meet performance expectations.
• Travel as needed to provide services.
• Work collaboratively with staff, management and community partners.
