Job summary

Family & Children’s Services seeks an RN Care Manager in the OKC/Norman area to deliver integrated nursing and case management services for individuals with behavioral health, substance use, and medical needs.

Responsibilities include assessments, direct nursing care, care coordination, and health education in collaboration with interdisciplinary teams to support recovery and improved health outcomes.

Qualifications

• Licensed Registered Nurse in Oklahoma.
• ADN required; BSN preferred.
• Experience in behavioral health or substance use disorders preferred.

Responsibilities

• Provide professional nursing care to individuals with serious mental illness, substance use disorders, and co-occurring conditions.
• Provide case management services and conduct nursing assessments.
• Administer and monitor prescribed medications.
• Coordinate nursing care with treatment team to ensure continuity of care.
• Engage clients and families in nursing education, care coordination, and adherence to treatment plans.
• Maintain documentation per organizational policy and regulatory standards.
