Family & Children’s Services is seeking an RN Care Manager to deliver integrated nursing and case management for individuals with behavioral health, substance use, and medical needs in the OK area. You will assess, provide nursing care, educate, and coordinate with inter disciplinary teams to support recovery.

Ideal candidates hold Oklahoma RN licensure (ADN required, BSN preferred) and demonstrate trauma-informed, patient-centered care while navigating community resources and ensuring

Qualifikationen

• Must be a licensed Registered Nurse in Oklahoma.
• ODMHSAS Case Manager II Certification within 60 days of eligibility.
• Valid driver license with acceptable driving record for client transportation.

Aufgaben

• Provide professional nursing care to individuals with behavioral health and co-occurring conditions.
• Provide case management services and nursing assessments.
• Administer and monitor medications per orders.
• Coordinate care with treatment teams to support continuity and safe transitions.
• Educate clients on health, medications, relapse prevention, and wellness.
• Document assessments, interventions, and outcomes per policy and regulations.
• Engage clients and families in education and care coordination as appropriate.
