NorthCare in Oklahoma City is seeking a Behavioral Health Case Manager II to coordinate outpatient care navigation for adults, children and families. You will assess needs, develop care plans, and connect clients to housing, transportation, education, and employment services to support recovery.

The role works from a community-based setting, requires ODMHSAS criteria, and travel to client homes. You will collaborate with a multi-disciplinary team and monitor outcomes while maintaining timely

Formación

• Must meet ODMHSAS CM II criteria.
• Options include high school diploma or GED with 36 months in mental health/substance use care, 60 college credits with 12 months’ direct care, or a bachelor’s/master’s with 6 months’ direct care.
• Bachelor’s or master’s degree in any field meets criteria with 6 months direct care.
• Must obtain ODMHSAS Case Management certification within 60 days of hire.

Responsabilidades

• Provides integrated services that are welcoming, engaging, and accessible in physical, behavioral health and social needs.
• Promotes trauma-informed care and patient-centered approach.
• Coordinates care across multiple treatment settings and collaborates with clients, families, and community partners.
• Maintains clinical records and documents progress toward treatment goals.
• Travel to client homes and community-based settings as needed.
