Job summary

Qualifications

Responsibilities

• Provide integrated services that are welcoming, engaging, accessible and in the community, addressing physical, behavioral health and social needs.
• Coordinate care across multiple treatment settings and support population management staffing and planning.
• Conduct assessments, gather information on symptoms, history and needs, and develop a care management plan integrating physical and behavioral health needs.
• Maintain clinical records timely and ensure compliance with policy, funding, accreditation, and productivity expectations.
