Job summary

Community Bridges, Inc. (CBI) seeks a Navigator II to enhance client success by conducting outreach, engagement, and comprehensive case management, connected to wraparound services. The role requires documenting interactions per AAC, ADHS/DBHS, CARF, and CBI policies within a multidisciplinary team.

The Navigator II will guide individuals with barriers to access housing, healthcare, and community resources, and map next steps for ongoing support and service access.

Qualifications

  • Highschool diploma or GED required; related degree preferred.
  • Minimum six months recovery from substance use or mental health disorder.
  • 1–3 years of healthcare-related work experience preferred but not required.
  • 1–3 years of recovery-oriented service preferred but not required.
  • Minimum 90 days experience in a Navigator or Peer Support role.

Responsibilities

  • Conduct outreach, engagement, case management, and wraparound supportive services.
  • Identify and reduce barriers; connect clients with needed services and resources.
  • Document and track interactions per AAC/ADHS-DBHS/CARF policies and procedures.

Community Bridges, Inc. (CB I) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

Job Summary

The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator II will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.

Skills/Requirements:
  • Highschool diploma or GED is required. Associate degree (or higher) in a field related to Behavioral Health is preferred but not required of this position.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
  • 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
  • Must possess a current valid driver's license. Must be able to provide a 39-month MVR.
