Job summary
South County Health seeks a Social Worker to coordinate discharge plans in collaboration with the multidisciplinary team in our Emergency Department. You will facilitate referrals for dispositional levels of care and provide behavioral health options, securing inpatient beds and authorizations when required.
The role includes arranging outpatient mental health/addiction referrals, supporting inpatient floors, and guiding families to community resources with a focus on patient-centered care and
Qualifications
- MSW preferred; licensed in RI.
- Minimum one year psychiatric experience.
- Experience with adult mental health and substance use.
- Proficiency with computers and EHR.
- Experience in community health or chronic care management.
Responsibilities
- Coordinate discharge plans in collaboration with the care team.
- Provide referrals to patients and families for dispositional levels of care.
- Identify behavioral health treatment options and secure inpatient beds as needed.
- Arrange outpatient behavioral health and addiction referrals.
- Assist with placement and community resources; conduct behavioral health consults as needed.
